Works closely with the Director and Counselor to develop and implement the MHCF Perinatal Substance Abuse Grant. Is a part of a multidisciplinary team of healthcare workers. Participates in performance improvement and continuous quality improvement (CQI) activities.
DUTIES AND RESPONSIBILITIES:
Directly provides and coordinatesservices for MHCF Perinatal substance abuse/behavioral health patients.
Meets with and works with patients on Substance Abuse and Depression
Consults and collaborates with providers/Counselor on care of patients
Ability to perform grant-required parameters.
Establishes care coordination and support resources for patients and families
Maintain Grant Database in collaboration with Perinatal Counslor, complies statistical reports
Works with OB provider offices in collection of data and coordination of services
Able to establish local connections to support grant-funded program.
Works with Montana Department of Health and Montana Healthcare Foundation to ensure program aligns with state and national treatment and recovery standards.
Knowledgeable of community resources available for patients and families.
Facilitates patient access to communit resources as appropriate.
Communicates appropriately and clearly with supervisor, manager, co-workers, other departments and physicians.
