Meets with and works with patients on Substance Abuse and Depression |
Consults and collaborates with providers/Counselor on care of patients |
Ability to perform grant-required parameters. |
Establishes care coordination and support resources for patients and families |
Maintain Grant Database in collaboration with Perinatal Counslor, complies statistical reports |
Works with OB provider offices in collection of data and coordination of services |
Able to establish local connections to support grant-funded program. |
Works with Montana Department of Health and Montana Healthcare Foundation to ensure program aligns with state and national treatment and recovery standards. |
Knowledgeable of community resources available for patients and families. |
Facilitates patient access to communit resources as appropriate. |
Communicates appropriately and clearly with supervisor, manager, co-workers, other departments and physicians. |
Maintains patient records; compiles statistical reports. |
