The Broward VA Outpatient Clinic in Sunrise, Florida is looking for an Advanced Registered Nurse Practitioner to work in the Mental Health and Behavioral Sciences HUD-VASH (Enhanced Team). The NP must be certified to perform the responsibilities in the area of assigned practice.The ARNP assigned to the MH&BS Service is immediately accountable to the Medical Director of the assigned unit/clinic/program, the Chief, Psychiatry Service and ultimately, to the Associate Chief of Staff for MH & BS. PLEASE NOTE THIS ANNOUNCEMENT IS OPEN TO CURRENT PERMANENT VA EMPLOYEES ONLY, ALL OTHERS SHOULD APPLY UNDER THE ANNOUNCEMENT CBTC- 11173654-21-TGP*** The HUD-VASH (Enhanced Team) is a community support program, utilizing an Assertive Community Treatment (ACT) model, targeted to deliver services for veterans struggling with chronic homelessness and mental illness. Services are designed to improve the overall outcome to obtain & maintain housing, improve quality of life through initial evaluation and management, supportive psychotherapy and referral to appropriate mental health service. The APRN will serve as a liaison connecting veterans with chronic MH issues to specialty MH services i.e. OSAC, PRRC, PRRTP, MHICM, etc. Psychiatry Services delivered by PMH-NP certified AP Participate in HUD/VASH interdisciplinary clinical rounds, review medical records, provide input related to specific MH concerns and document recommendations according to policy. Identify patients at risk for adverse outcome related to nonadherence to MH treatment. Track patients' progress while enrolled in the HUD/VASH program. Provide psychiatry intervention when clinically indicated and desired by patient. Conduct office/telephone/VOD visits when appropriate. Conduct field visits for at risk patients with a frequency of at least once monthly, with increased visits during crisis situations. A critical time intervention model is used, with recently housed veterans needing the most services and maintenance phase veterans needing only routine monthly visits. Link patients to community resources and VA programs in a patient advocate role. Monitor healthcare service delivery and educate patients regarding health maintenance. Provide Psychiatric Evaluation & Management visits (E&M) for HUD/VASH veterans. Ensure interdisciplinary communication with MH, Voc. Rehab, RT and SUD services as desired and clinically relevant to the veteran. Other duties include but are not limited to: Demonstrates practice level beyond an individual unit/team to encompass a program, service or service line. Uses sound clinical judgment in assessing, planning, implementing, documenting, and evaluating patients with mental illness Guides HUD/VASH staff in appropriate psychiatry treatment interventions, frequency and follow-up recommended for optimal outcome. Provides leadership during emergency situations. Anticipates and recognizes medical/psychiatric emergencies and initiates and leads others to implement appropriate and timely actions. Demonstrates ability to function effectively as a team member of the HUD-VASH program. Demonstrates ability to identify, evaluate and implement processes to improve clinical practice. Demonstrates independent ability to determine additional interventions and education needed. Demonstrates knowledge of quality improvement techniques and performance improvement process. Identifies opportunities for improvement and initiates performance improvement process. Participates in performance improvement activities, demonstrating leadership in unit and/or program level performance improvement initiatives. Serves in leadership role in interdisciplinary opportunities to improve patient care at the unit or workgroup level. Improves/Maintains quality care through patient/family education, incorporating performance improvement findings into practice, and promoting customer service/satisfaction. Documents data related to assessments, care provided and outcomes in the medical record. Performs, documents and incorporates Measurement Based Care (MBC) in the interdisciplinary treatment plan. Authorized specific privileges as assigned such as, performing histories, exams, ordering tests, etc. Non-Routine Duties/Practices as assigned, such as compiling administrative workload reports, QM reviews, research etc. Work Schedule: Monday - Friday 8:00am - 4:30pm; flexibility may be required to ensure the needs of the facility are met.
