The RN Care Manager is a registered professional nurse who is accountable for the provision of client care at the Finger Lakes Healthcare System. The Care Manager is directly responsible to the Nurse Manager of their specific service. The work locations are the Finger Lakes Healthcare System. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Experience with acute care and triage (particularly phone triage) preferred. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical patient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. ["The RN Care Manager uses the nursing process and evidence-based practice to collaborate with the Teamlet (Patient, Primary Care Provider, RN Care Manager and Clinical Support Technician) and larger Team (family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient-driven holistic care plan for life. The RN Care Manager is responsible for collaborating with services internal and externalto the VA to facilitate care transition in order to effectively meet the patients' needs. The RN Care Manager provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. The RN Care Manager demonstrates leadership by serving as an advocate for patients, team player to colleagues as she/he continues to enhance his/her own and the team's professional growth, development and practice. Meets all mandatory requirements for assigned area and will perform activities that reflect the educational, experiential, and competency requirements outlined in the 4 Dimensions and the criteria identified under each Dimension. The RN performs, reports, and records assigned nursing care in the Primary Care Clinics as described below and any other duties assigned by management. RNs are cross-trained to cover Community-Based Outpatient Clinics in the local area and occasional travel to the CBOCs may be required. Additional duties include COVID-19 screening and testing, and assignment to vaccine clinics as needed. Triages walk-ins appropriately. Uses appropriate telephone etiquette for both internal and external customers. Uses telephone assessment skills to identify actual and/or potential health, safety, and educational needs of the patient. Appropriately uses triage decision support based from the Telephone Triage Protocols for Nurses by Briggs to systematically assess and address caller needs. Appropriately prioritizes the urgency of caller needs utilizing critical thinking skills and decision support tools. Develops a collaborative plan of care with caller, their support systems, and their provider. Completes documentation for clinical calls and TRMs within 24 hours of call. Enters as ordered, locates as needed, and obtains results of x-rays, tests, procedures, and labs. Engages and documents patient education. Incorporates knowledge of ethical and legal issues and professional nursing standards necessary for providing patient care and information. Knowledge of and implementation of PACT principles. Completes hospital discharge follow-up calls within two business days and UCC/ER visit calls. Documents hospital discharge and post discharge UCC/ER calls in CPRS. Utilizes secure message as a means of communication with patients and documents pertinent health information in CPRS. Responds to secure messages within 3 business days. Facilitates and acts as provider in RN clinic including providing knowledge/information to patient/family to promote self care of acute/chronic disease management. Applies clinical knowledge of primary health care in prevention and wellness, health risk assessment, disease management, and self care. Adheres to proper infection control procedures regarding equipment, cleaning, and storage. Understands responsibility for the safe processing of reusable medical equipment. Monitors exam rooms and clinic frequently for cleanliness and patient presence. Follows policies for insuring the correct patient is receiving the correct treatment by using two patient identifiers. Performs procedures as direction, i.e. EKG, Doppler, l&O catheterization, venipuncture, PPD application, nebulizer treatments, dressing changes, vital signs and weights, etc. Administers medications as ordered and documents appropriately. Documents clinical reminders. Adheres to the Missed Opportunities Policy for cancellation of provider/Med RN appointments. Follows policies/procedures developed by their service, facility, and VISN. Facilitates flow of patients through clinic as team facilitator by assigning tasks/roles to RNs LPNs, Health Technicians, and MSAs.
