Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Current BLS provider card Several years of RN experience, with recent RN experience in a surgical setting (outpatient clinic, inpatient unit, and coordination) Ophthalmic Nursing experience Case Management experience Certified Case Manager (CCM) Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Physical Requirements: This position requires heavy lifting, 45 pounds heavy carrying, 45 pounds straight pulling (2 hours); reaching above shoulder; use of fingers; both hands required; walking & standing 8 hours - intermittently; repeated bending; both legs required; ability for rapid mental& muscular coordination simultaneously; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; depth perception; ability to distinguish shades of colors; hearing (aide permitted). Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. Is clinically and administratively responsible to the RN Manager or the Chief of Surgical, Interventional, and Procedural Services in collaboration with the Chief of Ophthalmology Eye Care Service. The major duties and responsibilities include, but are not limited to: CLINICAL: Provide routine patient care and management in the Ophthalmology Clinic setting.\nTriage patients with acute and chronic complaints, in consultation with Ophthalmology clinical staff and according to standardized procedures.\nRequest and follow up on diagnostic studies as indicated per request of Ophthalmology providers.\nCase management for patients with complex needs to coordinate and facilitate their Ophthalmology care (e.g. consultation for referral patients from outside of VAPAHCS and telehub patients receiving multimodal therapy, integrating care with other medical specialties).\nCoordinate clinics to ensure preop/cataract e consults and post-up follow up visits are scheduled in appropriate clinics with appropriate studies requested by Ophthalmology providers.\nCoordinate with social worker regarding patient lodging, meal, and transportation needs for appointments, treatments, and surgeries.\nParticipate in inpatient surgical care as needed to serve as a resource for patients and families for peri-operative and post-discharge care.\nEducate patients and their families as needed.\nServe as clinical resource person for other nurses, interdisciplinary team members, and other services regarding clinical issues concerning Ophthalmology patients.\nNotify supervising provider regarding patients with atypical findings or problems that are new or not resolving as expected.\nInitiates arrangements for hospital admissions and discharges and completes appropriate paperwork.\nAs directed by the supervising physician, enrolls patients in investigational studies approved by the Investigational Review Board (IRB), and orders the necessary tests. Completes mandatory training requirements to participate in research as needed.\nOrdering laboratory or other diagnostic studies in accordance with specific protocols. ADMINISTRATIVE & ADDITIONAL DUTIES: Prioritizes, coordinates and maintains the weekly operating room schedule to include: contacting patients, arranging for pre-operative work- ups, ensuring all pre-operative medical clearance is complete, arranging for appropriate peri-operative care and arranging for lodging reservations as needed.\nCoordinate telephone calls and Secure Messaging for Ophthalmology surgical section. Provide timely triage and review by appropriate provider.\nContact patients who No Show to pre-op/post-op clinic appointments and ensure appointments rescheduled as appropriate to maintain continuity of care.\nAssist with completion of disability, Family Medical Leave Act and Compensation and Pension paperwork at patient request.\nAssist with submission of patient lists for multidisciplinary clinical conferences (e.g. radiology, tumor board).Assist in the administrative closure of clinic encounter forms in CPRS as necessary.\nMaintain patient Data bases, if applicable.\nMaintain clinical competency through attendance at relevant continuing education programs and involvement in professional organizations.\nIncumbent schedules outpatient clinic appointments using Veterans Health Information Systems and Technology Architecture (VistA) scheduling options. Follows nationally established business rules to schedule appointments for veterans. This includes clinic visits, consultations and entries into the Electronic Wait List (EWL). EMERGENCY DUTIES:\nInstructed in treatment of life-threatening situations where a physician is not immediately available.\nInitiate emergency care as appropriate and makes every effort to summon a provider as soon as possible.\nProvide emergency treatment as directed by patient's request as noted in the advanced directive listed in DHCP/VISTA.
